The Association of Family Health Teams of Ontario (AFHTO) is a not-for-profit organization dedicated to supporting team-based primary care in Ontario. They work to promote best practices, share lessons learned, and advocate on behalf of all primary care teams, including Family Health Teams and Nurse Practitioner-Led Clinics. Evidence and experience demonstrate that team-based comprehensive primary care leads to better health outcomes and value for patients.

AFHTO's vision is to ensure that all Ontarians receive timely access to high-quality and comprehensive primary care that considers social determinants of health, involves a mix of health professionals working collaboratively with patients and caregivers, and is integrated into an equitable health system.
