The APH ConnectCenter Transition Hub is a one-stop resource for individuals with visual impairments and blindness, offering a wide range of services and resources. This includes career and employment support for job seekers and professionals, as well as information aimed at families, teachers, and counselors. The Transition Hub also provides comprehensive education resources for those with complex needs, eye conditions, and vision loss. In addition, the hub offers access to the APH Directory of Services, webinars, webinar documents, and a glossary of commonly used terms in the field.

Furthermore, the Transition Hub partners with organizations such as the Association for the Education and Rehabilitation of the Blind and Visually Impaired (AER), Vision Serve Alliance (VSA), and Perkins School for the Blind, Perkins Compass Program to provide additional transition services and support.
