## Description

The Continuing Care Hall of Fame (CCHoF) is an organization dedicated to recognizing and honoring individuals and organizations whose significant, innovative contributions have advanced Continuing Care Retirement Communities (CCRCs), also known as Life Plan Communities, and Continuing Care at Home (CCaH) programs. Its mission celebrates people and organizations that have helped shape these forms of continuing care through leadership, vision, and achievement.

CCHoF maintains a Hall of Famers program featuring charter members and induction classes, with honorees recognized in years including 2015, 2017, 2019, 2021, 2023, and 2025. The organization also presents induction events and makes highlights and videos from its ceremonies available, sharing the stories and contributions of its members.

The Hall of Fame emphasizes purpose, passion, and perseverance as qualities common among its inductees. It honors leaders, visionaries, analysts, strategists, motivators, and mentors whose work inspires others and leaves a lasting impact on continuing care. Through recognition and event programming, CCHoF preserves the achievements of those who have contributed to the development of CCRCs and CCaH programs.
