## Description

Chota Community Health Services (CCHS) is a nonprofit community health center serving Monroe County and surrounding areas in East Tennessee. Founded to make quality medical care accessible close to home, CCHS aims to provide affordable care and build lasting relationships between patients and their healthcare teams. Its medical-home approach coordinates medical, behavioral, and other needs, including referrals to specialists, diagnostic services, and community resources.

Services include family practice, same-day and sick visits, preventive care, pediatrics, geriatrics, immunizations, physicals, chronic-condition management, and health education. Behavioral health offerings include counseling and substance use disorder treatment. The center also provides on-site laboratory testing, X-rays, DEXA scans, and mammography through a local partnership. Patient support includes a sliding-fee discount program, insurance enrollment assistance, prescription assistance, and interpretation and translation services.

CCHS operates clinics in Madisonville, Tellico Plains, and Vonore, as well as school-based clinics in Monroe County schools. Additional services include employment, workers’ compensation, DOT, and drug-screening exams.
