訪問看護ステーション こころは、東京都江戸川区に位置し、「命をつなぐ・心をつなぐ」という理念のもと、多様な訪問看護サービスを提供しています。私たちの目的は、患者さんが自宅や施設で安心して生活できるよう支援することです。医師と連携し、身体的ケアや医療行為を行うことはもちろん、患者さんやご家族とのコミュニケーションを重視し、心理的なサポートも行います。

提供するサービスは、介護保険・医療保険に基づく訪問看護、自費訪問、ターミナルケア、リハビリテーション、精神科訪問看護など多岐にわたります。訪問看護が必要な方には、年齢に関係なく適切な支援を行い、安心して生活できる環境を整えています。また、相談は24時間受け付けており、土日も訪問可能です。皆様の「こころ」を尊重した看護を心がけています。
