Cordia Health is a Greek health initiative supporting people affected by chronic conditions through connection, knowledge, and practical help. Its mission is to ensure that no one faces illness alone, with a focus not only on physical health but also on people’s emotions, resilience, and ability to live on their own terms. The organisation is beginning its work in Greece with heart conditions.

Its main offering is the Cordia App, a digital community space for people living with heart disease.
