## Description

Disha Community Care is an NDIS and aged care support provider serving people across Greater Sydney and parts of New South Wales. Its approach centres on flexible, personalised care shaped around each person’s routines, preferences, goals and changing needs. The organisation supports people to manage everyday life, maintain independence and stay connected with their communities.

For NDIS participants, services include daily living and household assistance, personal care, social and community access, transport assistance, support coordination and recovery coaching. The team can also help people understand their NDIS plans, funding and plan reviews, and offers referral support for individuals, families and professionals. For older people, Disha provides help at home with personal care, meals, household tasks and daily routines, as well as community and lifestyle support for shopping, appointments and social activities. It also assists with My Aged Care applications and transitions home after hospital stays.

Disha works with families, carers and representatives where appropriate.
