Welcome to the Health Consortium of San Gabriel Valley! We are a diverse community of healthcare organizations, providers, and agencies working together to build and strengthen networks and partnerships in our service area. Established in 2000, our mission is to strengthen the healthcare safety net and ensure seamless access to high-quality care for physical health, mental health, and substance use disorder services in the Greater San Gabriel Valley. Our service area stretches along the foothill communities from Pasadena to Pomona and includes areas both north and south of the 10 Freeway.

We are dedicated to serving vulnerable populations, supporting access to services, promoting strategic partnerships, and increasing visibility for our participants. At the Health Consortium, we offer a range of resources and reports, as well as capacity building programs and initiatives. We host conferences and events to bring together healthcare professionals and provide opportunities for interdisciplinary collaboration.
