## Description

Healthy Chesapeake, Inc. Our mission is to support, sustain, and advocate for a healthier community. We are committed to addressing the declining health conditions in our area through collaborative efforts and evidence-based strategies. Our journey began in 2015 when a group of representatives from the City of Chesapeake, Health Department, Eastern Virginia Medical School, and Chesapeake Regional Healthcare came together to tackle this challenge. Led by Dr. Nancy Welch and retired Fire Chief Steve Best, we established Healthy Chesapeake as a hybrid organization. Driven by data and community needs assessment, we work towards becoming the healthiest city in Virginia. We believe that health and well-being are influenced by various factors such as social, cultural, economic, and physical aspects. As the designated population health manager for the Chesapeake Health Department, we implement Virginia's Plan for Wellbeing. Through our voluntary Coalition and Board of Directors, we collaborate with over 20 agencies and community stakeholders to prioritize and address the health needs of our residents. Together, we strive to make a positive impact on the overall well-being of the Chesapeake community. Contact us today to learn more about our initiatives or to get involved in our efforts.
