## Description

Heart @ Home provides flexible, non-medical personal support services that help people live safely and comfortably in their own homes. Serving clients across Canada, the organization supports people of all ages, with particular attention to seniors and individuals with disabilities who want to maintain independence. Its services can also give family caregivers time to run errands, attend to other commitments, or take a break.

Support is tailored to each client’s needs and may be arranged as weekly visits or on-demand assistance, from a few hours to continuous, 24-hour care. Services include companionship, personal care, bathing and grooming assistance, light housekeeping, shopping, errands, transportation and escorts, medication reminders, overnight care, and caregiver respite. Care may be short-term, such as after a hospital stay, or ongoing.

Heart @ Home begins with a consultation and in-home visit to identify the support required. A Client Care Manager then selects a suitable caregiver and arranges an introduction, with efforts made to provide consistent caregivers and backup coverage when needed.
