## Description

The IMPORTANT Project is a European research initiative focused on improving treatment decisions for older people with advanced hormone receptor-positive, HER2-negative breast cancer. Its name stands for “Implementing Geriatric Assessment for Dose Optimization of CDK 4/6-Inhibitors in Older Breast Cancer Patients.” Coordinated by Örebro Läns Landsting, the project brings together partners to study whether vulnerable or frail patients can begin treatment with a lower dose of CDK 4/6 inhibitors alongside endocrine therapy, rather than the recommended full dose.

The project is conducting a multicentre, open-label, prospective, randomized non-inferiority clinical trial using a pragmatic, decentralized approach. Comprehensive geriatric assessment informs evaluation of patients’ frailty before randomization. The study examines prognosis, treatment tolerability and quality of life, while also considering side effects, financial impacts and healthcare costs. Its work includes developing more accessible recruitment methods, supporting shared decisions tailored to individual patients, and preparing a geriatric cancer registry using assessment data and validated quality-of-life questionnaires.
