Its approach centers each young person’s strengths, preferences, voice, and hopes, while helping families navigate the many decisions involved in transition. The aim is to turn complex, uncertain questions into shared conversations and manageable next steps. At the heart of the planned offering is the Family Transition Blueprint, a living record intended to bring together a person’s qualities and goals with family intentions, important decisions, support, and future actions.
