Founded by social worker Chloris Leung, the organisation draws on lived experience and a commitment to social justice to make support systems clearer and more accessible. Its approach centres on hope, resilience and empowerment, helping participants build confidence, independence and skills to pursue their goals. Services include NDIS support coordination, specialist support coordination for people facing complex needs or barriers, and psychosocial recovery coaching for participants with psychosocial disability. The team also provides social work and capacity-building support focused on wellbeing, relationships and everyday challenges. Support may include understanding and using an NDIS plan, connecting with providers, coordinating services across areas such as health and housing, advocacy, and developing practical skills and long-term support networks. Services are available in English, Cantonese and Mandarin, with professional interpreters offered for other languages and dialects. ## Industries - ⛑ Health - ⛑ Mental Health _(under Health)_ - ⛑ Developmental and Physical Disabilities _(under Health)_ ## Links - [facebook](https://facebook.com/profile.php?id=61578970106503) - [instagram](https://instagram.com/ocean.hope.support) ## Logos & icons - logo _(primary)_ — PNG — [download](https://cdn.brandfetch.io/idDIAVlzGL/w/1118/h/1118/theme/dark/logo.png?c=1bxid64Mup7aczewSAYMX&t=1793636357612) - icon _(primary)_ — JPEG — [download](https://cdn.brandfetch.io/idDIAVlzGL/w/100/h/100/theme/dark/icon.jpeg?c=1bxid64Mup7aczewSAYMX&t=1793636357811) ## Colors | Name | Hex | Theme | | --- | --- | --- | | Surfie Green | #107582 | accent | | Juniper | #627f84 | dark | | Tumbleweed | #daab81 | light | ## Fonts - ui-sans-serif — asset _(custom)_ --- _Source: _ _See [/llms.txt](/llms.txt) for a full list of agent-readable pages, and [/auth.md](/auth.md) for how agents authenticate._
