Options for Community Living, Inc. Established in 1982, Options was created to address the need for housing in the community for people with serious mental illnesses. Today, they offer a range of supportive housing opportunities for individuals recovering from mental illness and provide Medicaid case management services for those living with HIV/AIDS.

As a Health Home partner, Options coordinates care for over 1000 adults and children with chronic illnesses annually. With over 2,400 adults and children served each year, Options manages more than 160 residential properties across Long Island. They are accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF), reflecting their commitment to delivering high-quality services.
