## Description

Project Access New Haven (PA-NH) is a community-focused organization working to improve access to medical care and health-related social services for underserved residents of Greater New Haven, Connecticut. Its mission is to connect people in need with care, coordinate timely services across the healthcare system, reduce health disparities, and support more equitable and innovative healthcare delivery.

PA-NH’s programs address a range of needs. The Urgent Specialty Care Program provides patient navigation and connects eligible residents with free specialty and ancillary care through a donated-care network. The Breast Health Navigation Program helps coordinate timely diagnostic testing and treatment for people with abnormal breast cancer screening results or a breast cancer diagnosis. Pressure Check is a research project piloting approaches to improve hypertension control, including support from community health workers. Through Integrated Care for Kids (CT InCK), the organization also supports a child-centered model focused on prevention, early identification, and treatment of children’s behavioral and physical health needs.
