## Description

Parker Jewish Institute for Health Care and Rehabilitation is a voluntary, not-for-profit organization in New Hyde Park, New York. Founded in 1907, it has grown from a shelter for 25 people into a health care and rehabilitation provider for adults. Its mission is to deliver high-quality care with compassion and dedication, while continually improving its programs and contributing to health care delivery and education.

Parker’s services include skilled nursing, long-term care, sub-acute care, and short-term rehabilitation focused on helping patients regain independence and return home. The organization also provides palliative care, comprehensive community hospice, certified home health care, and community health services. Through Parker at Your Door, patients can access services such as physician house calls, telemedicine, diagnostic testing, and chronic care management. Additional offerings include dialysis, vascular services, and volunteer support through Willing Hearts, Helpful Hands.

Parker supports patient and family education and participates in teaching and research. It is Medicare- and Medicaid-certified and accepts third-party insurance.
