## Description

Pitt Partners for Health (PPH) is a community health improvement partnership working to improve the health of people in Pitt County, North Carolina. Established in 1995, it brings together representatives from local churches, businesses, communities, and human service agencies to identify and address pressing health needs. Its mission is to improve population health through coalition building and partnership.

PPH coordinates community efforts through assessment, resource identification and development, citizen advocacy, comprehensive planning, and the coordination of health intervention and prevention strategies. Much of this work takes place through action teams organized around priorities identified in the Community Health Needs Assessment. Current teams focus on Healthy Lifestyles, Access to Care, and Mental Health, supporting initiatives intended to respond to the community’s identified needs.

The partnership also shares information and opportunities for community involvement through its website, which features news, newsletters, a blog, a calendar, meeting minutes, and a gallery. PPH is based at Vidant Medical Center in Greenville, North Carolina, and works collaboratively with community partners to advance local health improvement.
