## Description

PRAPARE® is a nationally standardized initiative aimed at addressing health disparities by equipping healthcare providers and community partners with essential tools to understand and respond to the social determinants of health (SDOH). Developed by the National Association of Community Health Centers (NACHC) in collaboration with notable partners, PRAPARE® facilitates the systematic collection of data regarding patients' assets, risks, and experiences. This data is crucial for improving health equity and making informed decisions at individual, community, and systems levels. 

PRAPARE® offers a variety of services, including a comprehensive screening tool, an implementation and action toolkit, and a resource center designed to support providers in utilizing this information effectively. With translations available in over 25 languages, the initiative aims to engage diverse communities and foster collaboration among healthcare providers, community organizations, and vendors. By participating in PRAPARE®, stakeholders can actively contribute to efforts that improve population health outcomes while addressing the underlying socioeconomic factors that influence health.
