Primary Health Tasmania (Tasmania PHN) is a non-government, not-for-profit organization dedicated to connecting care and promoting the well-being of Tasmanians. As one of the 31 Primary Health Networks (PHNs) established by the Australian Government, our mission is to enhance the efficiency, effectiveness, and coordination of medical services, particularly for those at risk of poor health outcomes.
