## Description

PRISM (Promotion des Réseaux Intégrés des Soins aux Malades) is a Geneva-based association that has developed ideas and activities around integrated healthcare since 2009. It focuses on improving outpatient care for people with complex needs, particularly those living with chronic conditions. PRISM’s work responds to the growing health, human and financial challenges associated with chronic illness and the fragmentation of care caused by medical specialisation.

The association aims to maintain high-quality care while involving patients in decisions about their treatment. It works closely with healthcare institutions, public health bodies, medical professionals, community services and patient partners in Geneva and beyond. Together, they coordinate care across professions and organisations through meetings that bring patients, their relatives and relevant professionals—such as family doctors, home-care nurses, social workers and physiotherapists—around the same table.

PRISM also supports the documentation of decisions in a shared electronic care plan, with a shared medication plan intended as a further development. Its approach promotes collaboration, communication and more integrated support for patients.
