## Description

RKM Primary Care is a 501(c)(3) nonprofit network of Federally Qualified Health Centers serving surrounding communities since 1999. Its patient-centered medical homes coordinate care for the whole person, with a focus on providing quality, affordable health care regardless of a patient’s ability to pay. The organization’s services include medical, behavioral health and dental care, along with women’s health, pediatrics, school-based behavioral health, remote patient monitoring, mammograms and pharmacy services. RKM also offers patient support options such as appointment requests, patient portal access, billing information, records requests and transportation services. Patients can find health centers across locations including Baton Rouge, Clinton, Jackson, Livingston, Loranger, West Baton Rouge, Slaughter and Springfield. The organization also directs visitors to community-based resources for needs such as food, transportation, housing and safety, and provides information about help enrolling in Medicaid or Medicare. RKM’s network receives funding from the U.S. Department of Health and Human Services and has Federal Public Health Service deemed status for certain health-related claims.
