Sacramento Covered is a community-based nonprofit organization that is committed to ensuring healthcare access and care coordination for individuals residing in the Sacramento region. Their mission is to enhance the overall health and well-being of the community by connecting people to a wide range of health services, behavioral health support, social services, and various resources. The team at Sacramento Covered is dedicated to making a positive impact across the continuum of care.

Comprising professionals with extensive training and passion, including 27 Field-Based Community Health Workers, 20 Health Navigators, 10 Patient Navigators, 3 Housing Coordinators, 2 Licensed Clinical Social Workers (LCSW), and speakers of 8 languages including Farsi, Dari, Pashto, Russian, Ukrainian, Hmong, and Spanish. They prioritize serving the most vulnerable members of the community and meeting individuals where they are on their healthcare journey. Notably, 27% of clients prefer a language other than English, 32% are non-US citizens, and 60% identify as Black, Indigenous, and/or People of Color (BIPOC).

A staggering 85% of clients assisted were eligible for Medi-Cal. With a dedicated board and a team of passionate individuals, Sacramento Covered continues to work tirelessly to improve the overall health and well-being of individuals and communities, ensuring everyone has access to crucial healthcare services
