## Description

Sage Support Coordination is an NDIS support provider based on Wadawurrung Country in southwestern Victoria, Australia. The organisation helps participants, families and carers understand their goals and circumstances, navigate the NDIS and connect with supports suited to their needs. Its approach is person-centred, built on respectful relationships, honest communication and guidance tailored to each participant.

Sage offers Support Coordination to help people understand their NDIS plans, engage appropriate services, build capacity and resilience, address crises and prepare for plan reviews. For participants facing greater complexity or barriers to accessing services, Specialist Support Coordination provides a higher level of assistance to coordinate supports and address challenges across their support environment. Sage also provides Psychosocial Recovery Coaching for participants whose psychosocial support needs affect daily life.

The team works with people from diverse backgrounds, cultures and life stages, with a focus on meaningful outcomes and supports that align with individual goals. Sage invites prospective participants and their supporters to get in touch to discuss their circumstances and next steps.
