## Description

Southeastern Minnesota Area Agency on Aging, Inc. (SEMAAA) helps older adults and their families age successfully, with dignity and informed choices. Serving Dodge, Fillmore, Freeborn, Goodhue, Houston, Mower, Olmsted, Rice, Steele, Wabasha, and Winona counties, the organization connects people with information, guidance, programs, and community resources that support independence and well-being.

Through Minnesota Aging Pathways, specialists provide information on Medicare, healthcare providers, home healthcare, long-term care, mental health, prescriptions, and hospice. They also offer free resource coordination for older adults who need support staying in or returning home after a hospital or nursing home stay. SEMAAA shares guidance on independent living, planning to remain at home, housing changes, finances, insurance, and long-term care fraud and abuse. Caregivers can receive help exploring available options and programs.

The agency also promotes evidence-based health education, age-friendly communities, and services for older adults. Its work includes supporting providers and community partners, administering funding opportunities, and sharing publications and resources related to the Older Americans Act.
