## Description

SickSlip is an online service that connects people seeking absence documentation with a physician for a medical evaluation. Customers complete a short intake describing their situation, and Dr. Adam Z. Kawalek, MD, personally reviews each case. He is a board-certified internal medicine physician with more than 15 years of clinical experience and licenses in 29 states. When a review supports issuing documentation, SickSlip provides a signed doctor’s note, typically the same day. Notes are designed for work or school absences and may address situations such as illness, travel, jury duty, or a mental-health day. Each note includes a QR code and unique document ID so employers or schools can verify its authenticity, while medical details remain private. SickSlip charges a flat $29.99 and offers a full refund if a note cannot be issued. The service also provides a superbill that customers may submit to insurance or use for HSA or FSA reimbursement. Its related services include sports and diving clearances, medical-necessity letters, and gym membership cancellation letters.
