## Description

SonarMD is a digestive healthcare company that helps care teams support people with Crohn’s disease and ulcerative colitis between clinician visits. Its platform uses monthly health check-ins, delivered by secure email or text, to track symptoms and identify potential disease flares. Responses are converted into personalized Sonar Scores, and the platform monitors changes over time. When a score or trend signals concern, SonarMD’s care coordination team contacts the patient and shares relevant details with the practice so clinicians can respond with appropriate outreach or care.

The service is designed for health plans, risk-bearing organizations, independent gastroenterology practices, and health systems. It aims to support more proactive care, reduce avoidable hospitalizations, and help providers manage chronic gastrointestinal conditions without adding routine monitoring tasks for practice staff. SonarMD also offers patient resources, including introductory guides to inflammatory bowel disease, lifestyle articles, and help using the platform. The company reports patient engagement and cost and utilization outcomes, with results published in peer-reviewed journals, including Gastroenterology in 2024.
