South West Diabetes Care supports people living with diabetes across South West Victoria, offering individualised education and practical guidance to individuals and families. Based at the Lyndoch Medical Hub in Warrnambool, the service also provides telehealth appointments.

Care is led by Claire Timms, a Registered Nurse and Credentialed Diabetes Educator with 16 years of experience in diabetes management. Claire works with children and adults and specialises in diabetes technology and insulin pump therapy. Services include support for Type 1, Type 2 and gestational diabetes, insulin pump starts and continuous glucose monitoring.

The approach recognises that each person’s experience, lifestyle and goals are different. Support may cover blood glucose management, insulin use, nutrition and the demands of daily life, with an emphasis on empathy, patience and encouragement. South West Diabetes Care aims to help people and their families build the knowledge, confidence and practical tools to manage diabetes. The service also assists clients in navigating diabetes technology and related information, helping them make informed choices about their care.
