TCOM, or Transformational Collaborative Outcome Management, is a dynamic approach founded on a unified vision aimed at enhancing health and wellness across varied sectors, including healthcare, child welfare, juvenile justice, and education. Established in 1998 by Dr. John S. Lyons, the Praed Foundation underpins TCOM’s mission to foster personal transformation in human services, with a particular focus on the well-being of children and families. 

The foundation offers essential tools and assessments, such as the Child and Adolescent Needs and Strengths (CANS) and the Adult Needs and Strengths Assessment (ANSA), ensuring they remain publicly accessible to all who share its commitment to collaborative care. TCOM emphasizes the necessity of cultural responsiveness, engagement, and respectful interaction in all assessments and interventions. By promoting collaborative processes and informed decision-making, the foundation strives for consensus in action, facilitating positive change and integration among diverse stakeholders.
