TCP Healthcare, also known as The C.A.R.E Professionals, provides expert clinical care, education and support for older people and those who care for them at home or in aged care facilities. Its services also support NDIS participants and care teams. Founded by Brian Tait, a nurse practitioner with more than 20 years of nursing experience and a specialist background in wound management, the organisation offers clinical consultancy, education, home care and complex case management.

Services include assessments and ongoing support in collaboration with GPs, along with guidance on wound care, catheter care and other clinical needs. Home visits provide tailored care and practical education to help families build the skills needed to support a loved one. The C.A.R.E Program helps strengthen care teams through consultancy, detailed care plans, training and continuing support, delivered face-to-face or by telehealth. Participants may also access a client portal featuring clinical resources, guides, webinars and useful links.
