## Description

Connected Communities is an initiative bringing together national, state, and local innovators to help neighborhoods build lasting, community-driven systems of health and social support. Its work focuses on older adults and people with disabilities, including those living with dementia, while promoting well-being across the wider community. The initiative encourages local leaders, advocates, housing providers, and long-term services and supports organizations to collaborate on a continuum of care that reflects local character and individual needs.

Connected Communities promotes person-directed support, self-determination, equity, and relationships as foundations of care. Its approach includes connecting residents with joyful activities, social events, transportation, green spaces, and neighborhood amenities, while empowering care partners through education and training. It also works to reduce operational silos between aging, disability, housing, and support services.

The initiative is developing interactive tools covering design, finance, governance, and performance management, expected to be available in early 2027. Its ongoing Connected Communities map highlights organizations, programs, agencies, and facilities that prioritize person-centered care and offer a continuum of support.
