## Description

Think About It is a public service initiative that helps people consider and communicate their preferences for care during advanced illness or disease. Its online form is designed to make conversations about end-of-life care easier to begin and to help reduce the pressure on loved ones who may otherwise need to make difficult decisions without knowing a person’s wishes.
