Trilogy Care helps people manage their Support at Home funding with greater choice, transparency and control. Its services are designed for individuals and families seeking to arrange care at home while making their funding go further. Clients can choose a Self-Managed or Fully Coordinated option, retain trusted carers, compare pricing and select services and providers that suit their needs, without restrictive agreements or hidden costs.

Trilogy Care supports clients through care planning, budgeting and service setup, and helps guide them through the aged care process. Depending on the chosen service, support includes a dedicated care partner or coordinator, care plan reviews, access to nursing staff and clinical escalation support, invoice payments, reimbursements, monthly statements and workforce compliance checks. Fully Coordinated services can also include sourcing, screening, onboarding and scheduling care workers, as well as coordination for people with complex needs.

The Trilogy Care Portal lets clients view their care plans, services, budgets and invoices online.
