## Description

VIA Health Partners is a nonprofit provider of hospice, palliative, and related supportive care, serving 49 counties across North and South Carolina. Operating since 1978, the organization focuses on comfort, quality of life, and respect for each patient’s choices, while guiding families through serious illness and end-of-life care. Its hospice services are available wherever patients call home, including private residences, assisted living and long-term care communities, and hospitals. For patients who need acute, around-the-clock support, VIA also operates six hospice houses offering care in a peaceful, home-like setting.

VIA’s palliative care helps people living with serious illness manage symptoms and maintain quality of life, with support for their loved ones. Specialized programs provide tailored care for people with dementia, children, and veterans. The organization also offers grief support, caregiver and family resources, and home-based primary care. Services are available around the clock, every day of the year. In addition to patient care, VIA invites community members to volunteer, attend events, donate, or pursue careers with its team.
