## Description

The Visiting Nurse Association of Greater St. Louis, now part of SSM Health, has served families in the St. Louis community for more than 100 years. Known for compassionate end-of-life care, the organization supports patients and families with services that address emotional, social, and spiritual needs alongside practical care.

Its offerings include hospice care, delivered by an experienced interdisciplinary team that works with families to develop care plans reflecting the wishes of patients and caregivers. The VNA also provides private duty care at home through its Care at Home services, with personalized one-on-one support available from four to 24 hours a day, seven days a week. Assistance can include medication management and reminders, bathing and hygiene, light housekeeping and laundry, companionship and safety supervision, respite care for people with dementia, and help with errands and transportation. These services are designed to help people live safely and independently at home.

The VNA’s Visit-A-Bit program connects older adults in the Greater St.
