WNY Integrated Care Collaborative (WNYICC) is the leading Community Care Hub in Western New York. As the hub of a Social Care Network, WNYICC partners with local community-based organizations (CBOs) to provide a wide range of evidence-based programs and services. Our mission is to help participants manage their health and enhance their quality of life.

Through our collaborative efforts, we aim to make partnering between CBOs and healthcare easier and more efficient. By centralizing operations at our Community Care Hub, we focus on addressing the Social Determinants of Health, closing gaps in care, and improving patient compliance with healthcare provider recommendations.
