Primary Care Collaborative (PCC) is a non-profit, multi-stakeholder membership organization that strives to enhance the healthcare system through the promotion of effective and efficient primary care and patient-centered medical home. Comprising various public and private organizations, PCC's mission is to bring together diverse stakeholders to advocate for comprehensive and integrated healthcare that treats the whole person. By unifying these stakeholders, PCC aims to achieve the Quintuple Aim: better care, better health, lower costs, greater joy for clinicians and staff, and greater health equity.

PCC was originally founded in 2006 as the Patient-Centered Primary Care Collaborative (PCPCC) and later rebranded as PCC in 2019 while still emphasizing the benefits of high-performing primary care for patients. PCC's work includes advancing research, care delivery and payment models, as well as promoting policies that enhance primary care. Their resources include publications and webinars, which disseminate information and outcomes from primary care and medical home initiatives.
