Project Transitions is a brand dedicated to providing home, health, and hope for individuals impacted by HIV in Central Texas. Since 1988, Project Transitions has been the only direct provider of housing for people living with HIV in the region. By offering stable housing, their services not only improve individual outcomes but also contribute to community health by reducing medical costs and preventing the further spread of HIV.

Their comprehensive support services include supportive housing, hospice, and recuperative care. With the generous contributions of donors and volunteers, Project Transitions is working towards building new, affordable, and supportive housing in Austin specifically for individuals living with HIV. This housing will feature wrap-around support services to ensure medication and treatment adherence, moving us closer to an HIV-free Austin.
