ABC Bemmel biedt specialistische begeleiding met ervaren coaches die zich richten op het ontwikkelen en vergroten van de persoonlijke en sociale zelfredzaamheid. Dit doen we door actief te werken aan de doelen die door cliënt en eventueel netwerk worden beschreven in de ondersteuningsvraag. Op basis van de ondersteuningsvraag stellen we een zorgplan op en starten we een traject passend bij de cliënt en zijn/haar hulpvraag. Het doel is om cliënten zodanig te ondersteunen dat we tijdens het traject kunnen afbouwen in intensiteit. “We mogen een bepaalde periode met iemand meelopen op zijn/haar pad en geven inzicht, vaardigheden en tools mee om, met ondersteuning van netwerk, zelfstandig verder te gaan.” – Olga We werken vanuit een integratief kader waarbij we per casus onderzoeken en overleggen op welke wijze we de begeleiding toepassen en welke ondersteunende methodes, tools en middelen we hierbij inzetten. We maken hierbij gebruik van systemische, oplossingsgerichte, psycho-motorische en cognitieve gedrags interventies. Daarnaast maken we veel gebruik van sport, bewegen en natuur
