APEX Health Services

APEX Health Services is a locally owned and operated home care provider serving Chicago and communities throughout Illinois. Established in 2008, the organization supports patients and their families with individualized care designed to help people live safely and comfortably at home. Its services include skilled home health care, nursing and therapy; non-medical assistance with daily activities; and private-duty care, with options such as companionship, meal preparation, and housekeeping.

APEX also provides hospice and palliative care focused on comfort, symptom relief, and quality of life, as well as transitional care for patients returning home after a hospital or facility stay. Specialized offerings include wound care and support for people managing chronic conditions such as diabetes, heart failure, COPD, cancer, and Parkinson’s disease.
