L'Association Autour Du Patient 94 est un regroupement de professionnels de santé de Nogent-sur-Marne, Bry-sur-Marne et Le Perreux-sur-Marne. Elle a pour objectif de réunir les professionnels du soin afin de faciliter l’accès aux soins, de favoriser l’échange entre professionnels autour des patients et de faire émerger des projets pour améliorer la coordination des soins de proximité des patients sur ce territoire. C’est une CPTS (Communauté professionnelle territoriale de santé).

About CPTS Autour du Patient 94
CPTS Autour du Patient 94 is an association of healthcare professionals serving the areas of Nogent, Bry, and Le Perreux-sur-Marne. As a Community Professional Territorial Health Association (CPTS), their mission is to bring together care professionals, establishments, healthcare facilities, and associations to improve access to healthcare, promote collaboration among professionals, and develop projects that enhance the coordination of local patient care. Their services include finding a primary care physician, non-scheduled care, prevention and screening programs, and specialized care pathways for conditions such as diabetes and cancer.
They also offer resources, tools, and support to help individuals maintain their health and stay at home. For healthcare professionals, the association provides opportunities to become members, access online resources, and advertise professional listings. Adhering to a patient-centered approach, CPTS Autour du Patient 94 supports both healthcare professionals and patients in their journey towards better health and wellbeing
