Hospice of the Valley

Arizona Supportive Care (AZSC) is a home-based medical support program serving central Arizona since 1977. A program of the not-for-profit Hospice of the Valley, it helps people living with chronic or serious illness manage symptoms and the challenges of daily life. Care is provided where patients live, including private homes, assisted living communities, and skilled nursing facilities, and is coordinated with each person’s physician. Patients can continue pursuing curative treatment with their own doctor while receiving AZSC support.

The multidisciplinary team includes physicians, nurses, nursing assistants, chaplains, and social workers. Services include in-home physician or nurse practitioner visits, care coordination with primary care providers and specialists, and in-person nurse visits after hours. AZSC also offers round-the-clock clinical phone support for questions and urgent needs. Its compassionate care extends to family caregivers, offering practical and emotional support as they navigate a loved one’s illness. With weekday office hours and 24/7 phone access, Arizona Supportive Care makes professional support more accessible and convenient for patients and families across its service area.
