Bloom Care Partners

Bloom Care Partners LLC is a Tennessee-licensed provider of person-centered support for children and adults with disabilities, older adults, and their families. Serving communities throughout Middle Tennessee, the organization helps people live as safely and independently as possible, participate in their communities, and receive assistance tailored to their needs, abilities, preferences, and goals. Its approach emphasizes individual choice, skill-building, family collaboration, and meaningful community connection.

Services include personal support for everyday activities, respite care for family and other caregivers, supported living, semi-independent living, adult habilitation, and placement services. Support may be provided at home, in residential settings, or in the community, depending on the individual’s needs and authorized service plan. Bloom Care Partners also works with families, case managers, support coordinators, managed care organizations, healthcare providers, agencies, schools, and community organizations to coordinate services and referrals.

Licensed by the Tennessee Department of Disability and Aging, Bloom Care Partners offers services subject to eligibility, funding, authorization, and program requirements.
