Brooklyn Plaza Medical Center, Inc. (BPMC) is a licensed Federally Qualified Health Center serving Fort Greene, Crown Heights, North Central Brooklyn, and surrounding communities for nearly 40 years. Rooted in its local community, BPMC provides comprehensive, continuous care for patients of all ages, from infants and children to adults. Its services include family medicine, dentistry, obstetrics and gynecology, allergy and immunology, and podiatry, delivered by a team of experienced clinicians and medical staff.

BPMC also offers programs designed to address specific community health needs. These include Rising Heights, which provides HIV/AIDS health services; Medication-Assisted Therapy, connecting people with opioid-use concerns to treatment and ongoing support; and a 340B prescription drug program that helps patients access discounted or free medications through partner pharmacies. Community outreach includes pop-up screenings at centers, local organizations, and colleges across Brooklyn.
