Coast & Country Primary Care (CCPC) is an award-winning, not-for-profit organisation supporting communities across the Central Coast, Port Macquarie and Coffs Harbour. Established in 2001, it provides inclusive health, mental health, disability and community programs to help people access care and live well. CCPC is a registered NDIS provider and is accredited against the NDIS Quality and Safeguards Standards, with an ISO 9001-certified quality management system.
Its services include NDIS support coordination, psychosocial recovery coaching, home and community support, personal care, inclusive social activities, therapeutic supports and plan management. Mental health and allied health offerings include psychology, counselling, occupational therapy, dietetics, art therapy, transitional care and support for young people. Community and care programs include Health on the Streets, homeless outreach, Coast Nutrition, Aunty Jeans and Culture Connect, as well as after-hours GP clinics. The organisation also offers aged care services, including home and garden maintenance. Depending on the program, people may refer themselves or need a referral from a GP or other health professional.
