Centrica Care Navigators

Centrica Care Navigators supports individuals and families in Southwest Michigan as they navigate serious illness, end-of-life care, and grief. The organization offers guidance throughout each person’s journey, helping clients and loved ones find support suited to their needs and choices. Its services include home care, pain and symptom management, hospice and end-of-life care, and emotional and spiritual support. Families can access caregiver coaching, resources, and support, while people coping with loss can find grief services and information.

Centrica Care Navigators also provides dementia care and works with physicians, hospitals, health systems, and skilled nursing and assisted living centers. Its Story Catcher program and Centrica Journeys offer additional ways to engage with personal experiences and care. The organization welcomes volunteers and provides opportunities to contribute through giving and fundraising events. Through its Life’s Big Moments storytelling initiative, presented by NorthStar Care Community, it encourages conversations and planning around end-of-life care. Centrica Care Navigators serves Allegan, Barry, Berrien, Branch, Calhoun, Cass, Eaton, Kalamazoo, St. Joseph, and Van Buren counties.
