Chota Community Health Services (CCHS) is a nonprofit community health center serving Monroe County and surrounding areas in East Tennessee. Founded to make quality medical care accessible close to home, CCHS aims to provide affordable care and build lasting relationships between patients and their healthcare teams. Its medical-home approach coordinates medical, behavioral, and other needs, including referrals to specialists, diagnostic services, and community resources.
