Community Health Provider Alliance (CHPA) is a nonprofit Accountable Care Organization (ACO) that is leading the transition to value-based care. Established in 2014, CHPA is a statewide integrated network of Colorado Federally Qualified Health Centers (FQHCs), also known as Community Health Centers (CHCs). They offer integrated primary care services to patients, regardless of their ability to pay.

CHPA leverages the strength of their member CHCs' integrated primary care model, which includes physical, behavioral, and dental services.

They also address the Social Determinants of Health (SDoH) through programs like payer enrollment, care management, patient education, and patient navigation. By partnering with various payers, CHPA has entered into value-based contracts to set quality goals and enhance resources for their member CHCs. They have ongoing programs and initiatives to improve care transitions, implement best practices, and advocate for payment reform.
