About Carina Health Network
Community Health Provider Alliance (CHPA) is a nonprofit Accountable Care Organization (ACO) that is leading the transition to value-based care. Established in 2014, CHPA is a statewide integrated network of Colorado Federally Qualified Health Centers (FQHCs), also known as Community Health Centers (CHCs). They offer integrated primary care services to patients, regardless of their ability to pay.
