CKZ Care, also known as CKZ Direct Primary Care, brings primary healthcare to patients at home, at work, or through telehealth. Based in Scottsdale, the practice serves Scottsdale, Phoenix, Cave Creek, and Fountain Hills, with evening, weekend, and after-hours appointments available by request. Its services include primary care, sick visits, COVID care and testing, telemedicine, house calls, and IV infusion therapy. The practice also offers complimentary weight-loss consultations and free consultations to help people determine which type of visit best suits their needs.

Care is led by Christina Karolzak, a board-certified Family Practice Nurse Practitioner with 32 years of nursing experience, including 18 years as a family nurse practitioner. Her background includes family medicine, geriatrics, pediatrics, and urgent care. CKZ follows a Direct Primary Care model, using a simple, flat membership fee rather than third-party billing. The approach emphasizes an ongoing relationship with a primary care provider, convenient access, minimal wait times, and longer appointments in person, by phone, or virtually.
