Collaborate Now is a family-owned support coordination provider based in Maitland, Australia, serving people with disability and their families across Maitland, Newcastle, the Hunter Region and the Central Coast. Drawing on more than 20 years of experience in aged care and disability, its team brings health, wellbeing, medical and legal backgrounds, alongside qualifications and lived experience. The organisation works with self-managed, plan-managed and agency-managed NDIS participants, and connects with registered and independent providers.
Its services cover three levels of NDIS support: Support Connection helps participants understand their plans and connect with providers and wider supports; Support Coordination assists them to design and build a network of formal and informal supports; and Specialist Support Coordination helps address complex situations, barriers and crises. The team also supports plan reassessments and changes in circumstances.
Areas of experience include dementia, acquired brain injury, muscular dystrophy, motor neurone disease, stroke, palliative care and supported housing.
