Community Connections – Te Hapori Awhina Tangata is a community-based service that is dedicated to providing support and services for young people and adults with disabilities, as well as their families. Operating in various regions including Wellington, Hutt Valley, Porirua, Kapiti, Horowhenua, Manawatu, Taranaki, the Bay of Plenty, and the Hawke’s Bay area, our main focus is on ensuring a person-centered approach. Our goal is to empower individuals to live the life they choose within their own community.
Through our range of services, such as Supported Living, Residential Support, and Neighbourhood Connections, we aim to provide the necessary assistance and natural supports for people with disabilities to live independently, connect with their community, and enjoy a fulfilling life. We believe in an inclusive New Zealand where barriers are removed and individuals with disabilities are fully embraced. Our vision is to honor people's stories, build capability in individuals and communities, and celebrate diversity and success.
